*NOTE: Some conditions may require a referral from a doctor, psychologist, psychiatrist or other licensed health professional. 

When do I need a doctor’s referral?
The laws of the State of PA require a doctor’s referral for any therapeutic, medical, psychological, or psychiatric issue. A standardized form can be emailed, mailed or faxed to you or directly to your doctor for signature. This means that hypnosis, subconscious expert skills part of Intueri Hypno TM service care management can help you with, for example, the stress underlying and resulting from depression, but in order to work directly on the depression, your doctor’s permission is absolutely required. If you are seeking care managment for pain a referral is mandatory.

When do I NOT need a doctor’s referral?
When you simply need to quit smoking, lose weight, be a better athlete- golfer, relieve stress, find a lost object, get motivated, or revisit a past life, and so forth, no doctor’s referral is required. This is in strict conformance with PA State law.

• * Childbirth Education Classes & Teleseminars. Under no circumstance is attendance permitted without consent form for all participants. If this is an enrollment for Childbirth Education Class and your partner plans to attend, each of you must complete a form separately. In-person workshops, we simply ask particpants to sign forms prior to start of series at seminar.
